Malignant Phyllodes Tumor of Accessory Breast -A Case Report-


Abstract
Accessory breast is a developmental anomaly which lies along the milk line or elsewhere, the most commonly in the axilla. Most benign and malignant mammary diseases in the accessory breast were described. However, phyllodes tumor occured in
accessory
breast has not been reported. We experienced a cytologic and hisopathologic findings of malignant phyllodes tumor in axillary accessory breast. The patients was a 28 year old lactating woman at seven months after delivery. Physically two
accessory
nipples were found in both axillary regions with right sided huge mass. Mammographically both breasts were normal. Fine needle aspiration from the right axillary mass revealed cellular smear of anaplastic spindle cells having large hyperchromatic
and
pleomorphic nuclei and prominent nucleoli in the necrotic background. In addition. There were scant amount of benign ductal epithelial cells forming sheets and clusters. Grossly, the resected mass was well circumscribed and 8¡¿8¡¿6cm in size. Cut
surface showd tan solid and lobulated appearance with mutifocal shallow clefts. Microscopic findings was that of typical malignant phyllodes tumor of the breast' high cellularity of stromal cells with frequent mitoses, pleomorphism and focal
necrosis,
in addition to benign ductal epithelial cells lining the cleft like spaces and small tubules which is occasionally found within the stroma.
